Stroke/Neuro Community Rehab Package

Service Description

Occupational therapy intervention in your own home or community setting aimed at facilitating you, and those who are supporting you, to come to terms with this life changing event. Having a stroke, or a neurological event/illness is life changing, even when considered a 'mild' event, this often causes huge changes to how you feel about yourself, and how you engage with your usual activities. This short term package includes 6 weekly home (or community) visits, education and support to adapt your lifestyle to reduce future stroke risk, introduction to mindfulness for stress reduction and rehab to achieve your short term goals. The primary focus, is to work on your mindset and adopt tools that can support you to continue your recovery. Top up sessions are available after completion of package.
